Applying for a U.S. Passport

It takes several working days to receive a new passport once you submit the application. We encourage you to check your passport for the expiration date and to apply early for renewal.

Everyone (including minors) seeking a U.S. passport must apply in person at the American Citizen Services office at Embassy Warsaw, Consulate General Krakow, or the Consular Agency in Poznan. The Consulate General in Krakow accepts passport applications by appointment only.

You will need to submit the following documents:

• Proof of Citizenship, such as a birth certificate or Certificate of Naturalization. At the discretion of the consular officer, other evidence, such as a photocopy of your lost or stolen passport, may be acceptable.
• 1 passport photo, 2 in x 2 in (5cm x 5cm), taken within the last six months. The photograph should be in color and must be front view, full-face taken in normal attire, and without a hat or dark glasses.
• The appropriate passport application form. The link for each passport application appears below.

Please be advised that the following passport fees apply, as of July 13, 2010:

• For renewal of an adult passport, 110 USD.
  Please complete Form DS-82 online.
• For a first adult passport, or for adults who previously possessed a 5 year children's passport, 135 USD.
  Please complete Form DS-11 online.
• For an emergency passport,  or for adults whose previous passport was lost, stolen or damaged, 135 USD.
  Please complete Form DS-11 online.
• For a child's passport, 105 USD.
  Please complete Form DS-11 online and see the Special Requirements for Children Under Age 16.
• For extra passport pages, 82 USD.
  Please complete Form DS-4085 online.
• For passport cards for adults, 55 USD; for children under age 16, 40 USD. Please complete the appropriate application form provided in the instructions.

Fees are in U.S. dollars or the equivalent in Polish currency, payable in cash or credit card (Visa, MasterCard, American Express, Novus/Discover, Diners). We cannot accept payment by check.
