» Special Requirements for Children Under Age 16
Children under fourteen must personally appear at the Embassy and have the consent of BOTH parents before a passport can be issued. If either parent cannot be physically present at the time the passport application is submitted, a signed and notarized » Statement of Consent of Issuance of Passport to Child Under Age 16 must be submitted together with a copy of the absent parent's passport.
If extreme circumstances exist which will prohibit obtaining the notarized consent of both parents, please contact the Embassy's or Consulate's American Citizens' Services Unit for further assistance.
Parents should submit, as proof of identity, photo identification, passport, driver's license, or other official government identity document. For proof of parentage, parents should submit the child's birth certificate (for Polish birth certificates, the odpis zupelny/"long form") listing the names of both parents.
